1. Agreement to Terms

By accessing our website, booking an appointment, purchasing a product or membership, or receiving services at Aloha 369 Massage & Wellness, you agree to be bound by these Terms and Conditions. If you do not agree to these terms, please do not use our website or services. These terms apply to all clients, visitors, and users of the Aloha 369 website and physical studio located at:

2. Services

Aloha 369 Massage & Wellness offers a comprehensive range of professional wellness services including but not limited to:

• Licensed medical massage therapy

• Deep tissue, myofascial release, craniosacral therapy

• Lymphatic drainage — pre and post surgical

• Hot and cold contrast therapy

• Facials, chemical peels, and microneedling

• Anti-aging skin care treatments

• Reiki and energetic healing

• Hypnosis therapy

• Yoga, breathwork, and guided meditation

• Quantum resonance bed therapy

• Wellness memberships

• Retail skin care and wellness products

All services are performed by licensed, certified, and trained professionals. Services are subject to availability and may change without notice.

3. Appointment Booking & Confirmation

Booking

Appointments may be booked online through our booking platform, by phone at (850) 936-5300, or by email at normakealmt@gmail.com. All appointments are subject to availability and are not confirmed until you receive a booking confirmation.

Confirmation

You will receive a confirmation via email or text message upon successful booking. Please review your confirmation carefully and contact us immediately if any details are incorrect.

Health Intake Forms

All new clients are required to complete a health intake form prior to their first appointment. This information is kept strictly confidential and is used solely to ensure the safest and most effective treatment possible. It is the client's full responsibility to disclose all relevant medical conditions, medications, allergies, and contraindications prior to receiving services.

4. Cancellation & No Show Policy

Cancellation

We respectfully request a minimum of 24 hours notice for all appointment cancellations or rescheduling requests. Cancellations made with less than 24 hours notice may be subject to a cancellation fee of up to 50% of the scheduled service value.

No Show

Clients who do not show up for a scheduled appointment without prior notice will be charged 100% of the scheduled service value. Repeated no shows may result in a requirement to prepay for future appointments or suspension of booking privileges.

Late Arrivals

We understand that life happens. However out of respect for all of our clients and therapists, late arrivals may result in a shortened session at the full service price. Sessions cannot be extended beyond the scheduled time when doing so would impact subsequent appointments.

Exceptions

We understand that emergencies and unexpected circumstances arise. Cancellation fees may be waived at the discretion of management in the case of a documented medical emergency or other extenuating circumstances.

5. Payment Terms

Accepted Payment Methods

Aloha 369 accepts the following forms of payment:

• Major credit and debit cards

• Cash

• Optum insurance — for qualifying services and eligible clients

• Gift cards issued by Aloha 369

• Wellness membership credits

Optum Insurance

Aloha 369 is proud to be an Optum insurance credentialed provider. Clients using Optum insurance are responsible for understanding their individual plan benefits, copays, deductibles, and coverage limitations. Any services not covered by Optum insurance remain the financial responsibility of the client. Please contact us prior to your appointment to verify your Optum insurance coverage.

Pricing

All service prices are subject to change without notice. Current pricing is available on our website and at our studio. Prices confirmed at the time of booking will be honored for that appointment.

6. Wellness Memberships

Membership Agreement

Wellness memberships at Aloha 369 are month to month agreements that provide clients with access to discounted services, priority booking, and exclusive member benefits. By purchasing a membership you agree to the following terms:

Billing

Membership fees are billed automatically on a monthly basis to the payment method on file. It is the member's responsibility to ensure their payment information remains current and accurate.

Cancellation

Members may cancel their wellness membership with a minimum of 30 days written notice prior to the next billing date. Cancellation requests must be submitted in writing via email to normakealmt@gmail.com or in person at our studio.

Freeze Policy

Members may request to freeze their membership for a maximum of 60 days per calendar year due to medical reasons or extended travel. Freeze requests must be submitted in writing prior to the next billing date.

Non Transferable

Wellness memberships are non transferable and may only be used by the registered member. Membership benefits may not be shared, gifted, or transferred to another individual.

Unused Credits

Unused membership session credits do not roll over beyond 30 days from the date of issue unless otherwise agreed upon in writing by Aloha 369 management.

7. Gift Cards

Purchase & Use

Aloha 369 gift cards may be purchased in studio or online and redeemed for any service or retail product offered by Aloha 369. Gift cards have no expiration date and are non refundable.

Lost or Stolen

Aloha 369 is not responsible for lost, stolen, or damaged gift cards. Lost or stolen gift cards will not be replaced or refunded.

Non Transferable for Cash

Gift cards cannot be exchanged for cash and have no cash value beyond their use as payment for Aloha 369 services and products.

8. Retail Products & Shipping

Product Sales

Aloha 369 offers professional grade skin care and wellness products for purchase in studio and online. All product sales are subject to our Refund & Return Policy available on our website.

Shipping

Orders are processed and shipped within the timeframe stated on our shipping page. Aloha 369 is not responsible for shipping delays caused by carriers or circumstances beyond our control. Risk of loss and title for products pass to you upon delivery to the carrier.

9. Health & Medical Disclaimer

The services offered at Aloha 369 Massage & Wellness are intended for wellness, relaxation, and therapeutic purposes. They are not intended to diagnose, treat, cure, or prevent any medical condition and are not a substitute for professional medical advice, diagnosis, or treatment. Always consult your physician or qualified healthcare provider before beginning any new wellness treatment, especially if you have a pre existing medical condition, are pregnant, or are recovering from surgery or injury.

10. Right to Refuse Service

Aloha 369 reserves the right to refuse or discontinue service to any client at any time for any of the following reasons:

• Inappropriate, disrespectful, or threatening behavior toward staff or other clients

• Failure to disclose relevant medical conditions that may contraindicate treatment

• Intoxication or impairment at the time of appointment

• Violation of our professional boundaries policy

• Any behavior that compromises the safety, comfort, or wellbeing of our team or clients

In cases where service is refused due to inappropriate behavior, no refund will be issued.

11. Professional Boundaries

Aloha 369 operates as a strictly professional therapeutic wellness studio. Any request for or suggestion of services of a sexual nature will result in the immediate termination of the session, full charge of the service, and a permanent ban from our facility. Aloha 369 reserves the right to report such incidents to the appropriate authorities.

12. Photography & Media

Aloha 369 may photograph or record our studio, events, and services for marketing and promotional purposes. By visiting our studio or attending our events you consent to being photographed or recorded in common areas. Clients will never be photographed or recorded during treatment sessions without explicit written consent. If you do not wish to be photographed in common areas please notify our front desk upon arrival.

13. Intellectual Property

All content on the Aloha 369 website including text, images, graphics, logos, and service descriptions is the intellectual property of Aloha 369 Massage & Wellness and is protected by applicable copyright and trademark laws. No content may be reproduced, distributed, or used without prior written permission from Aloha 369.

14. Limitation of Liability

To the fullest extent permitted by law, Aloha 369 Massage & Wellness, its owners, staff, and affiliates shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our services, website, or products. Our total liability to you for any claim arising from your use of our services shall not exceed the amount paid by you for the specific service in question.

15. Indemnification

You agree to indemnify and hold harmless Aloha 369 Massage & Wellness, its owners, employees, and affiliates from any claims, damages, losses, or expenses including reasonable legal fees arising from your use of our services, your violation of these Terms and Conditions, or your violation of any rights of a third party.

16. Governing Law

These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Florida. Any disputes arising under these terms shall be subject to the exclusive jurisdiction of the courts located in Santa Rosa County, Florida.

17. Changes to These Terms

Aloha 369 reserves the right to update or modify these Terms and Conditions at any time without prior notice. Changes will be posted on this page with an updated effective date. Your continued use of our website or services following any changes constitutes your acceptance of the revised terms. We encourage you to review these terms periodically.
